Our dietitians Clare Wolski, Rebecca Mete, Michelle Bulman and Melissa Yip are Accredited Practicing Dietitians with the Dietitians Association of Australia and look forward to meeting and helping your patients with their health goals.

It’s very easy to refer your patients to The Healthy Eating Hub.

Just download and print our dietitian referral form. It contains all the necessary information for your patient to make a booking with us, plus a section for some notes and information from yourself, if necessary.  GP Referral Form (338 downloads)

If your patient has a Chronic Disease Management Plan or a Team Care Arrangement (formally known as Enhanced Primary Care Plans), they will require a letter and the appropriate paperwork from you to accompany this referral form. Billing policy is written below.
